HOW TO REGISTER AS A PATIENT AT THIS PRACTICE.

If you wish to join our practice please ask the receptionist for a registration form for each person that wishes to join. The form will ask you for personal details such as your name, address, date of birth and your previous doctor’s name and address. This will assist in tracing your medical records as quickly as possible.

Once you have completed and signed the form you will be placed on Dr Powell’s list although you are free to see whichever doctor you wish. You will then be sent a new medical card with your doctor’s name on for you to keep. During the registration process the receptionist will make you n appointment with the practice nurse for a new patient check.

THE PRACTICE AREA

The Practice area encompasses the villages and towns of Hayfield, New Mills. Birch Vale, Thornsett, Rowarth, Furness Vale, Buxworth and Chinley.. We accept patients from parts of Whaley Bridge and Disley.

Appointments
Appointments can be made by telephoning 0844 4994470 for Hayfield or New Mills or by calling in at either surgery. Routine appointments can be made up to two weeks in advance which will enable us to offer you appointment at a time more suitable to your requirements.

We are now able to text you a reminder the day before your appointment if you are interested in this service please contact reception and give them your mobile phone number

Cancelling your Appointment
If you are unable to attend an appointment with one of the doctors or nurses, please telephone or use the link at the bottom of this page to cancel your appointment.

Home Visits
Patients are requested to telephone before 10.30am if a visit is required that day. These visits are intended only for those patients who are housebound or who are genuinely too ill to attend surgery. Emergency visits only will be arranged after that time
